2009 PFA Co-Ed Adult Volleyball Tournament

The 20th Annual Adult Co-Ed Volleyball Tournament will be held on March 13-14, 209 in St. Louis, MO, with Nest #45 as the hosts for the weekend.

All Instructors/Instructresses or anyone interested in entering the tournament should email Chris Puskar for a spreadsheet entry form. You will receive an entry form to complete and return to her via mail.

The following items are to be included in your spreadsheet:
- Name
- Please indicate whether a Nest of District team or the need to be added to a team
- Policy Number
- Gender
- Number of guests attending and dining with us
- Capatin's information as stated on the spreadsheet

All Entries Must Be Emailed & Checks Postmarked By February 28, 2009!

The entry fee is $55.00 per participant and $25.00 per guest. Checks should be made payable to Polish Falcons of America and mailed to:
