COVID-19 Relief Fund

Eligibility Requirements

  • The applicant must be an Individual Member who has been infected with the Coronavirus and has experienced a significant financial setback as a result of the Coronavirus.
  • The applicant can also be an Individual Member with an immediate family member who has been infected with the Coronavirus who has also suffered a financial setback. Immediate family members include a spouse, parents or in-laws, or someone who is dependent on the care of the Member.
  • The applicant must be a Member of the PFA in Good Standing (which means all National Dues much be current at the time of application.)
  • Grants are available one per household.
  • Applications will be evaluated on a case-by-case basis.

Documentation must be provided within 7 days of submitting this application. Please send documentation (i.e. doctor letter, unemployment claim, etc.) by email to cpuskar@polishfalcons.orgIf documentation is not received within 7 days, you will need to reapply for consideration.
